CareManage+ — SMS Terms of Service

These Terms of Service govern the Calvary Medical Clinic text messaging program (the “Program”), administered by CircleLink Health, LLC on behalf of Calvary Medical Clinic. By providing your mobile phone number and opting in, you agree to these Terms.

1. Program description. When you opt in, you will receive recurring automated text messages and calls related to the CareManage+, including enrollment invitations, appointment and care reminders, chronic care management, and related healthcare support from Calvary Medical Clinic.
2. Message frequency. Message frequency varies, but is typically no more than 3 messages per month.
3. Cost. Message and data rates may apply for any messages sent to you from us and to us from you. If you have questions about your text or data plan, contact your wireless provider.
4. Opt out. You can cancel the SMS service at any time by texting STOP to the number. After you send STOP, we will send a message to confirm that you have been unsubscribed, and you will no longer receive SMS messages from us. To rejoin, sign up again as you did the first time.
5. Help. For help, text HELP to the number, or contact us at support@circlelinkhealth.com.
6. Carriers. Carriers are not liable for delayed or undelivered messages.
7. Consent. Consent to receive text messages is not a condition of enrollment or of purchasing any goods or services.
8. Privacy. Your information is handled in accordance with our Privacy Policy. Mobile opt-in information and consent will not be shared with third parties or affiliates for marketing or promotional purposes.
